Keep Signature History in Progress Notes
I sometimes have to unsign progress notes in order to change the service location for secondary claims or re-submitting claims. When our therapists re-sign the notes it appears that they signed the note days, weeks, or even months after the appointment which causes them concerns about liability or audit issues. Why doesn't the note capture all signature activity history? Seems to me that when a note is unsigned a comment should be available to add to the date of unsigning and captured for the note history as well as the re-signing date. All historic activity and dates would then display…
