Allow multiple client signatures on intake forms
Allow me to add multiple client signatures to intake forms. I work with children who may have parents and stepparents and I want to be able to capture all of their signatures.

We will also like that the first signature does not lock the document as we will need to edit, goals and objectives as we are collecting the signatures.

Allow for multiple signatures on intake documents to include family. For example both parents and children over age 12.

Agencies require signature on all clinical documentation by the therapist and overseeing clinical supervisor. However, TheraNest only allows one signature per document. Therefore, I am suggesting the option to add MORE than ONE signature to each document such as progress notes and assessments, so that we are compliant with insurance and Medicaid requirements. Thank you.

I need to be able to have everyone who is in my room sign a consent and to have parents sign and intake with the child signature, etc. So if I have a family of one kid and two parents, I need two parents and a kid signature (if they are 14 years and up).
So as it stands now, I only have one parent filling out the forms. Then I have to have paper forms filled out with everyone else's signature and down load it to be compliant.

Can we add a second signature box for children over age 12 or a second parent on the online intake forms?

When working with clients who are minors, it would be best if at the end of the intake process on the client portal there was space for a second signature (the guardian's signature). I imagine this would also be helpful when working with couples.

please allow multiple e-signatures such as: one for the Supervisor/CEO, client, client's parents if minor and clinician

Allow for multiple signature options to be available on forms since most of the documents must be signed by Program Director or Supervisor and other forms require the parent's signature if the client is a minor

Please also make sure that we can have the option to print the signature lines so guardians and minors can sign paper copies of the forms if they do not want to sign them online!

Yes, also we need to be able to have both parents sign. So ideally we would be able to add two parents and the child or adolescent sign all the intake documents.

Allow for multiple signatures (on different lines) as in the state of WI, parent consent is needed for children under that age of 18 years. The current form does not allow for this.

Yes! Especially in states when minors can give consent to treatment and what info they do and do not want released. Or adults who are under guardianship - the guardian and the client can sign the document.

I think this was said better in a different request "Divorced parents being able to create & submit two sets of intake paperwork on client portal". They need to be able to sign in separately and complete the paperwork separately. I know there are technical ways to go about this, but CLINICALLY I can't ask them to share an email address/password just for this.

It would also be nice if dual reminders could be sent to parents (especially when children's parents are separated). Also if the client portal could allow a parent to schedule appointments/settle invoices, etc for multiple children who may be seen.

Some states (like Montana) require two parent authorization for mental health services. In order to use intake forms on the client portal, their needs to be a way to require two signatures and/or a check mark to to claim sole guardianship.

I would like to have more than 1 person be able to sign my forms. This would be helpful when seeing a family or someone under 18 year old.
