Allow multiple client signatures on clinical notes
Allow me to add multiple client signatures to clinical notes. I work with children who may have parents and stepparents and I want to be able to capture all of their signatures.

In cases of minor clients, it's often a requirement to have the consent of BOTH parents for treatment -- especially in cases where there is shared physical custody and decision making is 50-50. We need to be able to see which email address forms are being sent to as well as have the option to send forms individually or collectively to more than one person in the contact network.

I work with adults that have legal guardians. This would be super helpful to be able to have all signatures.

Yes, REALLY need. Most of our clients are telehealth right now and this feature would help tremendously with documentation. Right now all we can do is add a blurb about their signature represents both the child's and theirs and that they have discussed it with the child :/

Allow 2 signatures on the intake forms so both parent and a minor older than 12yrs old can sign the consent forms

Working with children, we often have two parents/guardians that we want to collect signatures from. The current forms only allow for addition of one signature block at the end of the forms, we would like to see two.

We do need this, but we also prefer to be able to submit paperwork to parents separately. Also considering couples, we prefer they each get a log in to the portal and complete paperwork in private and that not all consents are able to be viewed by the other parent or partner.

it is necessary for minors age 12 and up to be able to sign and their parents/guardians to sign as well.

On intake form there should be multiple signature boxes for both guardians (if applicable) and for pts who are age 12 and over (legally, a 12+ minor needs to sign the paperwork too).

I can't believe this was suggested back in 2018 and still has not been worked on. This is a great idea. We often have to either print the form or create another portal. It would be easier if for example one informed consent could have multiple signatures for different family members since everyone is required to consent to treatment but there is only one insurance holder.

Here's a real life example of Kathleen Bready's 9/16/2020 concern. Today, because of a chargeback, I have to send the client's "shared forms" intake documents (that is ALL of her intake documents - including the ones that have super personal information like what medications she takes) ostensibly to the client's BANK because each form does not have its own client signature. Because you only have ONE signature at the end of multiple documents, I can't break apart the document and only send the pertinent forms (informed consent and credit card consent forms). Developer/coder, imagine if this was YOU - would you want your bank - or anyone besides your therapist - knowing what medications you take, the psychological problem you're experiencing, your history, etc.? TheraNest, this fix needs to be prioritized!

When working with minors, a guardian must sign the plan. Some insurance companies require the child to also sign the plan if they are 14 years old or older.

Please look into adding a guardian signature to all forms… treatment plans, custom forms, etc. I work primarily with minors and have to have guardian signatures. This recently came up as a big issue when my files were reviewed by a payor. Historically I have had guardians sign under “client” showing the client’s name under the guardian’s signature.

Currently Dynamic Forms are only able to be signed by the patient, and/or by the Therapist (if completing in person). It would be nice to have the ability to send via the client portal and collect a patient signature, then also collect a therapist signature upon review. This would help as legal documentation to capture the review of the custom Dynamic Form.
