Autopopulate client information in custom forms and intake forms
When creating custom forms, it would be helpful to have the client information boxes be automatically added to the form, or to at least have the option to add them to the form. The same way would appear on progress notes and the initial assessments.

Hello!
Today we released a feature to Dynamic Forms called, Dynamic Fields! We started off with 5 fields for now and will look to UserVoice on what we should add next! Today you can add the fields: Client Name, Client ID, Date of Birth, Phone Number, and Email to your forms. These fields will auto-populate this information when used in Dynamic and Intake forms. We’re excited to grow this feature so please continue to contribute your ideas to push us forward!

It'd be nice when the client sends in their GAD7 and PHQ9 or whichever forms that the information gets autopopulated into their file tabs too. It's such a hassle to have to reenter everything by hand.

It would be ideal if the client name and employee name auto-populated on every custom form. Currently we are entering boxes for these fields into each custom form and it can be cumbersome. Thank you!

It would be great to have Consent Forms available to send to the Patient Portal at any given time throughout treatment with this feature and the option to sign.

It would be a great idea to have the intake forms information be auto populated to the clients detail page and insurance section. This would help with time management and ensure that there is no information missing.

Custom forms are able to pull or populate from anywhere in the Theranest...
For example my custom progress notes can populate from the information in Theranest as well as show up in the signed notes section to prevent having a report of notes to be signed that have already been signed.

I suggest that intake forms upload client information directly into client information/files. As it is now the client fills out the form, for some reason it is converted to a pdf and then I would have to go back pull up that file and then copy the information all over again after the client has supplied it. Other EHR's with client portholes do not require this extra step for me.

Could you have the client's intake information populate their psychosocial? As part of the intake process the client provides information on the reason for treatment, family, education, medication, etc. All of this is part of the psychosocial and could come directly from the forms they complete.

On customized forms, you should be able to pull from the list of client's names on the form, input the active staff name from the system, and pull up the client's diagnostic code on their case.

It would be helpful to have the ability to link fields from client portal intake forms into the custom forms that show up in client details. For example, if an intake form was to authorize my company to contact a previous clinician, that those fields could be added into a custom form on the client details page that would populate with that information.

Allow client information to be pulled into custom form templates, like name, address, essentially all the fields that are already being pulled from for billing and insurance forms.

Custom forms should automatically populate a clients Name, DOB and basic client details where needed on custom forms to prevent therapists having to re-type the clients details each time on each custom form.

Please give us more to work with on custom forms. Here are my suggestions:
1) Allow fields to be pre-populated based on information already entered (e.g. Clients Name, DOB, etc.)
2) Titles should be able to be used more than once (e.g. the ability to have check boxes saying "yes," "no," "N/A," etc. under multiple sections) I see several suggestions for this feature.
3) The ability to make only some sections into columns, and allow those columns to be seen while typing the custom form. Right now if you use columns the entire form has to be in columns, and not everything looks right in columns. Also, columns are only seen when printed. It would be nice to save some space instead of one long form, and have the formatting still look nice (e.g sections with multiple check boxes, or sections containing client info - name, DOB, etc as columns while large text sections remain alone)
4) It would also be nice to have the tabs to add fields on the side available as you scroll instead of only on the top or bottom - it can be quite tedious to go in and edit a long form.
