Formatting options for dynamic forms
Would like the ability to format custom forms similar to other word processors:
- Adjust font sizes, colors
- Paragraph styles
- Inserting tables
- Adding images

Integrate Docusign (they offer BAA), a Hellosign. Both allow you to enter form fields in a document (or PDF) that you create outside of Theranest and include signature lines. You can also create custom forms there as well but with more flexibility in terms of font size, bold, etc. Docusign will allow you to send a link to a client or have them complete form and sent to a secure encrypted email but I'm gathering if service is integrated then there's probably a way to have everything load to portal - perhaps... Anyway, I'd pay more for this service. Perhaps you could offer theranest with and without that as an option in terms of pricing.

I'd love to be able to customize the notes. This would include drop downs and ways to easily select what was covered during the session. This would allow a note to be completed within seconds after a session. Thanks!

It would be amazing if we could paste in a word document into the forms that could be filled out as a note by the therapist.

In order to increase accessibility for those with learning / reading differences, I would like the ability to do simple format changes and font changes to the custom and pre-written forms. It does not have to be fancy- but the ability to add spaces between lines, bold words, and change the font to one that does not "blend letters together" would be a huge and impactful improvement.

Add number fields, formula fields, fields that will pull info from existing data...to custom forms

I would love to see more format options. I agree with others with the usefulness of:
font size/colors
inserting tables
adding an image
adding a blank line for spacing to the next page
repeated text should be allowed (need client initials or signatures on multiple lines)
required choices which require text answers
It is also taking an exorbitant amount of time to create a custom form, which I types on my word doc in 10 minutes.
I am using a lot of paper and ink with the font at its current size.
Thank you! -

And to be able to have assessment measures automatically scored after the client completes them

I know this has been around as a suggestion for a long while and may not seem that important but really, it is. TheraNest...any updates?

Any update on this one yet?

Yes! And be able to insert a text box, then split it into columns within that box (rather than the whole page having to be 2 columns). This would allow for easier flow for symptom check boxes, etc.

Allow tables, custom font and formatting, and allow the use of same text repeatedly in a form, and fill-in blanks in a text block would all make for more user friendly and professional forms.

Give us the ability to create spreadsheets in the custom form section

I would love to be able to upload or copy and paste my existing templates and use them for each client. Working with the custom forms only allows for very basic text creation and is unnecessarily complicated and tedious, which is why I am still keeping my notes on a separate hard drive and just using word docs.

Allow repeats in heading sections

Allow formulas (akin to excel: index/match, if, count, sum, etc.) to be input and applied to items when creating custom forms. then we could also track add progress based on client responses to forms created, if so desired.

I consider myself pretty computer literate but find the custom forms section to be almost unusable . I would love to be able to create my own and just upload them.

HUGE need not only for intake forms but also for clinical forms. The options are way too limited right now. Either add options or integrate with an existing form vendor like Docusign or adobe. This is really limiting!

Be able to create custom forms with more flexibility in format, font, etc. More like a word-processing program.

Add numerical value (which calculates percentages) -

I want to create columns that doesn't require adding row numbers.
