Formatting options for dynamic forms
Would like the ability to format custom forms similar to other word processors:
- Adjust font sizes, colors
- Paragraph styles
- Inserting tables
- Adding images

The existing custom forms feature of Theranest is clunky and restrictive. No options for any kind of formatting. Google drive forms is a user friendly and flexible platform for building forms, and being able to seamlessly integrate client information from google drive into theranest would be a significant value-add in my book.

It would be amazing if the form feature was more developed. For example, able to set conditions and have more customization options. I currently use a 3rd party for all my intake forms. I would love to actually be able to use the forms feature in Theranest, but it doesn't have needed features.

I would love for the forms I am creating in the client portal to be even more customizable. Currently, I have a lot of questions that require a lot of checkboxes, and it would be nice if those checkboxes could go into 2 columns without the entire document being divided in 2. I also am trying to create a document that has a lot of text, and I cannot indent the text as needed or even put spaces in between paragraphs. When I am converting my very nice and neat paper documents to online documents, they appear so much more unorganized and confusing. I also need signatures on various documents, instead of just once, and I have information releases that I want as separate documents from the intake, because there could be several, depending on the client! Please make this more adaptable to the providers that have very unique intake processes!

Please give us more to work with on custom forms. Here are my suggestions:
1) Allow fields to be pre-populated based on information already entered (e.g. Clients Name, DOB, etc.)
2) Titles should be able to be used more than once (e.g. the ability to have check boxes saying "yes," "no," "N/A," etc. under multiple sections) I see several suggestions for this feature.
3) The ability to make only some sections into columns, and allow those columns to be seen while typing the custom form. Right now if you use columns the entire form has to be in columns, and not everything looks right in columns. Also, columns are only seen when printed. It would be nice to save some space instead of one long form, and have the formatting still look nice (e.g sections with multiple check boxes, or sections containing client info - name, DOB, etc as columns while large text sections remain alone)
4) It would also be nice to have the tabs to add fields on the side available as you scroll instead of only on the top or bottom - it can be quite tedious to go in and edit a long form -

Add a form element that is a horizontal spacer. Custom forms have terrible formatting when printed because all the fields are crammed together without any blank space for visual organizing or grouping. So add a form elements that allows for blank horizontal space when printed. Also allow for text formatting the the form elements text (headers, names of fields, simple text, etc.)

It would be very useful to be able to edit font types, sizes and colors on custom forms to be able to differentiate different sections of the custom form, and to be able to visibly see the difference between a text box title, and the information entered in a text box.

It would be helpful to have more from field types and a general redesign of forms creation and implementation. For example, the following field types and customization options would make the forms feature more useful:
data matrix
rating scale
Multiple Choice
signature Field
captcha
logic and branching (conditional fields)
ability to format text
basic field calculations
page breaks
a preview button to preview the form while creatingAs they are now, the forms a of limited use and I am finding that I need to rely on a third party solution (Jot Forms), which doesn't integrate with Theranest.

ability to choose different fonts and font sizes on all printed materials, reports, etc. the existing look is very unattractive and unprofessional looking.

I want to be able to collect behavior data within the record so to quickly add the date, if missed school, had suicide thought, cut self, etc. I want to be able to add new section to same form and not have one big text box

In custom forms, particularly- I often work with information that I need to track across columns and rows. a simple feature to insert a table would save me so much time in having to duplicate text boxes, etc., and would make it much less clunky if I wanted to create something like a log where I can see all the information visually on one page (as opposed to generating a new form each visit under a custom tab, etc.)

The note templates as is are not suitable for most insurance audits. They need to have Family, medical, social, legal, coping, existing and new problems, coping used since last session, symptoms check list

If the custom forms allowed for sub-columns under a section, it would make them much more user-friendly and save paper when printing for insurance company reviews. For example, if I have check boxes, I could:
Subjective
The patient was:
__Alert __Cooperate ___Non-cooperative
__Tardy __Drowsy ____AgitatedThis saves TONS of space and makes the note easier to write.

We agree. This needs to be done for all forms! Treatment plans, and notes especially. The coloring is too light when printed and the size of the print is too small. Please fix!

I couldn't agree more. Best case scenario would be if we could upload our own consent forms. I was able to customize the consent, but the signature function is seriously lacking- esp. for clinicians who treat minors. There needs to be a function for patient name and patient/ parent/ guardian signature.

It's not that rudimentary, but does need a little tweaking. I've created several forms that did take me a bit to figure out, but it does the job. I have to say that there are a ton of suggestions here and Theranest does a really great job at working on these requests. I'm fairly certain, this will get worked out as well.

And they need to be savable/organized by service date, not just created or saved date!

The current set up is non-functional, and rudimentary. There needs to be copy and past options, spacing features, and alignment options. Bold and Italicized options. There needs to be an upload function to upload already completed Informed Consents. Do something ASAP Please!

When creating a custom form it is very confusing I think having a format similar to word or google docs would be beneficial. Also, adding an option of a signature line would be a great idea because most forms request the treating clinician sign the notes.
