    This is especially useful when treating teenagers as you can send a reminder to the Pt and to the parent(s).

    It would be helpful to have more from field types and a general redesign of forms creation and implementation. For example, the following field types and customization options would make the forms feature more useful:

    data matrix
    rating scale
    Multiple Choice
    signature Field
    captcha
    logic and branching (conditional fields)
    ability to format text
    basic field calculations
    page breaks
    a preview button to preview the form while creating

    As they are now, the forms a of limited use and I am finding that I need to rely on a third party solution (Jot Forms), which doesn't integrate with Theranest.
