An error occurred while saving the comment In the state of AZ, we are required to have both the client and therapist’s signature on all intake consent forms. It’s great that both are added to the treatment plan, but it would be great if it could be added to the consent forms. I always have to download it and re-upload it with my signature after the fact.
In Arizona, per my licensure board/AzBBHE, we are required to have the future review date for the treatment plan listed. I noticed that there's already a target objective date option for each objective, however, that is not the same thing as the future treatment plan review date which is required by law in AZ. Currently, I have to manually write that in under the definitions section just so it'll show on the signed treatment plan.
