Add ASAM assessment tool

For those of use who deal with chemical dependency, it would be extremely helpful if there was an ASAM assessment tool we could use.

    I see that GAD 7, PHQ 2 and PHQ 9 have been loaded for access. Is there anything in the works for ASAM or ASI to be added?
