Customize existing clinical note templates
Allow me to customize the existing clinical note templates (i.e. progress notes, treatment plan, initial assessment, discharge summary) so that I can add and remove fields that are relevant to my practice and meet my payer requirements.

 commentedCan Treatment Notes (a custom form) fully replace Progress Notes? The error is: No diagnostic codes found in the notes for (date) session. Make sure the session has progress notes for the date and time of the session.

 commentedI like most of the progress note format, but it is missing some things. I tried to create a new template which did not work well either. Since most of what we need to submit for insurance is there, having a way to either add to the current note or use the note as a start to another template would be awesome. For example, having a place for those in attendance of session, with the client or being able to add a couple additional information boxes while keeping the remaining note items would be great.

      Shane Murphy commented This is very important and would also need to be efficient, like integrated with the progress note so as able to document this easily while documenting the progress note (vs. having to pop out back and forth to other windows or at least be able to open a second window while also in the progress note).

There was an article you put our regarding psychotherapy notes vs. progress notes and advertise your program, but I cannot find how you would do this using theranest...unless I am missing something. It would be great to add my more typed notes on insights for particular cases beyond progress notes. Thanks!

 commentedBranching/rules (e.g., client clicks on "yes" form unhides questions/fields, client clicks on "no" questions/fields remain hidden or client clicks "private pay" financial aggreement that requires signature is made visible, client clicks "bill insurance" fields related to insurance (i.e., policy #, name, group #, etc.) are made visible. Table/Matrix (singe entry - check boxes rating scale; multiple entry - column heading medication, dosage, times or people living home, name, age, relationship). Attaching/embedding consent forms or other forms that require signatures to intake forms.

 commentedI think it could be beneficial to allow therapists to adjust their note taking form- I get frustrated that I can't read the entire note at glance and have to scroll down to read it and while I'm typing. I also feel that the current form limits counselors to take notes in a specific way which may not be applicable to everyone!

 commentedI agree that I would like to see this be editable. I am a psychologist, but I don't want the fields in that order and I want other pieces of information in the form. Currently, we don't use the form at all. We use our own form and upload it.

 commentedThis really NEEDS to be optional. I understand the software is geared toward mental health, but there are a large number of users that are not in this field. Please at least allow for two choices of Initial Intake forms. Maybe one geared toward speech, physical, and occupational therapy and one for mental health. I really with the initial intake form could be altered more to fit my needs as a Speech Language Pathologist.
