Customize existing clinical note templates
Allow me to customize the existing clinical note templates (i.e. progress notes, treatment plan, initial assessment, discharge summary) so that I can add and remove fields that are relevant to my practice and meet my payer requirements.

      Kerrie Toole commented I need to be able to reformat the Therapeutic Intervention section to make check-boxes that are easy for my therapists to utilize. We are leaving So. Much. Money. on the table because they aren't confident in documenting in a text box but would be fine checking a box that listed interventions and rationale for 90837 codes, for example. And a pre-populated listing of standard Interventions would be ideal, just like what is done when you're building the Treatment Plan. It's already there, TheraNest!!! Just link it to the Notes!

I would love more personalization in general--I'm a sole practitioner and I really would like to be able to choose which fields I use on my progress notes and initial assessments.

      Elizabeth Benson commented I really wish that this was priority on TN's To Do list. It is disappointing and concerning that this was first suggested in 2014 and it is still not done. I just took continuing education class on this very topic last week and learned that there is SO much is missing from TN templates to be HIPAA compliant and to be ethically sound for insurance companies in order to to pass audits. This is really concerning to me. TN do you have any updates on when we could expect this?

 commentedSimilar to the ability to add/delete fields from the Progress Note form, it would be great if we could add fields that are more specific to substance abuse treatment. ASAM Risk and Level of care; Stage of change; DSM-IV diagnostic criteria for symptom severity, etc.

 commentedI would love this, especially as an LPC-A, since I don't use the majority of what's on these forms. It's very overwhelming to look at some of these pages with all this extra boxes I don't need. I don't know about anyone else, but it's a killer for my ADHD having to see all those empty boxes and trying to figure out which ones I actually need.

 commentedCurrently there is no area in the Assessment area to add support for the clinical formulation of the diagnosis. This requires me to create a separate form to provide written support as to the diagnosis as given to the client.

      Andrea Brandon commented In Simple Practice I was able to write my own "custom" progress note. They had this option by being able to select a blank screen from their drop down menu, as you could on your drop down menu of notes to select from. They also had the ability to have non clients schedule through their portal to receive services. I would like to see that service.

 commentedCreate space to include diagnostic history (including diagnostic provider as well as start and end dates). It would also be helpful to include rule outs. This needs to be able to be fluid and amendable throughout treatment rather than being locked via signature.

 commentedIt would be highly beneficial if clinical administration would be allowed to edit the initial assessment in TheraNest. There are different state standards for assessments and the initial assessment currently present in TheraNest does not meet criteria for our state's standards. Therefore, I had to create a whole new assessment. It would be beneficial to update the initial assessment - with adding requirements that we have. Thanks!

This would be truly beneficial for a number of clinicians, especially for time management." Couldn't agree with this suggestion more. It would be even better if we could do this with treatment plans and discharge summaries as well. Having a drop down option of choices for notes templates that we have created would also be helpful- for example, I use a different note format for my play therapy patients versus my adolescents and adults and would like to be able to create and save both to choose from as needed.
