Need the ability to bulk send forms, ie an updated consent form to all active clients for signature, or an outcome measurement to be sent out quarterly to all clients. As a extra bonus, it would be great if there was a way to track and interpret the data in reports.

This is a critical feature, in my opinion. We need to be able to track client progress, and screeners such as these are an important part of that. At another job, they used smart forms (which may be a feature of ECW), and the smart forms automatically created charts and graphs of all existing screener data so their responses could be easily tracked.

Hello! We have some dynamic forms and intake forms that we use to send clients clinical surveys to complete prior to their first appointment. It would be amazing if the system could calculate scores based on the numbers that the clients select in the measures for quick automatic scoring to save our therapist's time. Let me know if this could be an option!

Please add the scoring guide for DAST 10, AUDT-C, and CAGE. This is a barrier to efficiently serving clients.

Update the PHQ-9 and GAD-7 forms to total itself and indicate the severity scale based on the toal.

Example, if the score on the PHQ-9 is 17, the form will automatically indicate moderately severe depression next to the score.

It would be great to have a spreadsheet type interface for these screeners, so we can see progress over time.

As a workaround if you have a form you usually give you can create a note field in your template to capture this data. I agree what you are suggesting is much better.

On the GAD and PHQ it would be nice to have the total score on the input screen so you can see the trend without having to click into each assessment

This in conjunction with another user's request for making forms like PCL able to be completed electronically would be awesome. If there was a place for all these results to automatically pool and organize by date, that would be amazing. Even better if there was a shorthand way (like a dotphrase in EPIC) to import the last 3 scores for a particular measure into the note.

It would be awesome to have a spot in the client chart where we could indicate previous scores on checklists, and easily review. So- if they took the PHQ9, for example, we could add a line item with the date and score, and then update as we re-evaluate. It would be so much simpler to review progress made in relation to assessment, especially when we need to create records for outside requests (disability, workers comp, etc). Thank you!

Please consider adding the Adult Symptom Screener with score computation in the screening/intake options. This comprehensive screening tool was accessible in a prior EHR and automatically generated scores based upon responses entered. Multiple clinicians at our practice have used it, including me.

Is there a way another column could be added to the PHQ9 date & time screen list and GAD7 Scale list that gives the total score for that form on that day. For instance, currently when you click on the PHQ9 questionnaire link, it shows the date and time, edit, print and delete. Can another column be added that says 'Score'? That would enable us to see a running score and assess progress.

Can the PHQ-9 and GAD-7 have the scoring criteria on the bottom on the form?
