Resend Intake Forms multiple times throughout treatment
We need to resend our long term clients the intake forms again more than 6 month in service. As we know, by law, clients have to sign or update the intake forms again after 6 month. Sometimes we add a new service next to our counseling sessions, so we would like to be able to send another contract or new forms to sign via our client portal.

We have released a new feature to allow admin users to resend intake and client forms throughout treatment. From the client’s General Documents tab, you can share new forms and manage already-shared forms. New shared forms are accessible by your clients through the Client Portal (in the “Forms” menu option).

An option is to create an "Intake Form" in the Client Portal which solely serves as your Authorization to Release, Disclose, and/or Obtain Protected Health Information.

Hi, I have a client who completed an intake just under one year ago and the client portal is not letting her update anything. Some of these agreements expire within 1 year. If she has been a patient in the past, she will need to update some forms. I would like to see some option to update intake forms within one year (or after they expire within a year). Thank you.

Yes, I agree that it is important to be able to update intake forms. It seems like it would be an easy fix to allow users to re-send the intake packet and store each signed packet as a unique PDF by date signed.

Hi. I would love to have a separate form that clients can fill out for a Release of Information with a signature to be able to coordinate with other professionals. Right now if it's part of the intake, it's all one big pdf that I would have to edit to be able to send just the release of information and their signature with it. Looks suspect to another doctor (is this a cut and paste???). Would love to see functionality that this is a separate form they complete. Thank you!

I would like the ability to provide custom forms to clients through the client portal, such as Authorization to Release Information, that are not necessarily a part of the Intake Forms/process. In this way, all forms can be completed by the client through the portal and maintained electronically, without having to print out a form, have the client complete it manually, and then scan the form and upload it to the client's file.

We have added some items to our intake paperwork for clients to indicate what numbers/emails they consent for us to contact them as well as permissions to send thank you's to referral sources and indicate if they agree to us consulting about members of their family also receiving treatment. If they do not fill everything out appropriately before they e-sign, we are required to have them do it again in paper form. It would be great if they were able to go back in and try again.

It would great if there was an option to send clients custom forms via email, without attaching them to the intake forms. This could be used for simple assessments and surveys that need to be included in the client file but would be more accessible via online.

Lots of our clients either lose their paperwork on their email or don't finish it then can't get back in so they call us to send it again.

It would be nice to have the capability to copy "custom forms" to the "Intake forms" portal. I have created forms that I would have to recreate in order to make them available via the client portal.
