Customize existing clinical note templates
Allow me to customize the existing clinical note templates (i.e. progress notes, treatment plan, initial assessment, discharge summary) so that I can add and remove fields that are relevant to my practice and meet my payer requirements.

Intervention check boxes would be very helpful. Streamlining the note process in anyway would help me be able to be with my clients with fuller intent.

Allow for customization on assessments.

Can you design Theranest to allow providers to revise the initial assessment and diagnostic tabs as we see fit? The current design is not fully compliant with missouri medicaid standards, and if I could add the appropriate sections it would be perfect.

Would love to be able to add to the Initial Assessment form. We have to do a more detailed assessment for medicaid - the form there is a good start, but we need to be able to add custom fields!

Yes, please! I use the GIRP format for some clients, but that doesn't work for every type of client. For my EMDR clients, the current format is pretty awkward and un-relatable. I'd love to be able to edit the note format, and have thought this for 2 years. Please please please? Thanks!

If you could make the fields in the current Initial assessment and progress note customizable per user it would be very helpful. I am a psychiatrist working in theranest with a group of therapists and the current fields do not make sense for me.

Edit progress note template so that the user can add or delete fields that are not used. Creating custom forms will not automatically pull in information into the form like the date, dx, and tx plan. However, using the template just doesn't meet our needs. We need to be able to record weight, BP, and medication scripts.

either linking custom notes to schedule or ability to customize (or just add fields at a minimum) to progress notes...also there was another thread about this a year ago so you have many more people interested in this change than you think!!!

Provide option to differentiate progress notes from therapy notes; ability to link any note to schedule/agenda; customizable formats based on a practitioners needs; better functionality in documenting common symptoms and related goals and interventions by use of check boxes; ability to check boxes for interventions that align with DSM diagnosis; formats that provide note options that align with empirically supported theories.

Change the intake for or allow it to be changed. I know I am not the only one out there who has issues with it. No AOD section and the working Suicide Contract make it a harder form for me.

i think it would be great if you can reformat the progress note section. please make the mental status section narrower and widen the space where we write in. It will be aesthetically pleasing and easier to write in. Currently, the progress note space is very small and narrow

Please give us the edit option on your default forms! building a custom form is tedious and often it will not pull from the treatment plan/initial intake and it is very annoying.

It would be helpful if you could add NEGLECT to the list of Risk Factors on the initial assessment form.
Also, under the list of contacts, could you put AGENCY (like DHHS) as an option?

I hope this gets changed quickly! Our accreditation body wants to see certain fields in the tx plan and progress notes. Not having the ability to edit the pre-made forms is a HUGE hindrance on work flow.

I love Theranest and what it offers. With this being said, now want to say that I hope you really take a look at the initial assessment and add some things or let us be able to adjust it. You have no AOD section. The term contract for risk is very out dated. Not all of the boxes apply. I know there has been others suggesting this and I hope you are considering other options. I feel as though it is IMPERATIVE to be able to customize progress notes, given the wide range of practitioners using this system. For instance, I primarily do Psychological Assessment, and a huge chunk of the progress note is not applicable to testing appointments. At the very minimum, I'd like to be able to customize sections/headings to better meet my needs, but I also would like to be able to have a Testing-specific note type to use for most appointments. I'd be happy to provide a sample document to the company so you can make a psychological assessment note type AND have it be associated with an appointment like a progress note is.
I'd also like to see the intake/history form be something that is actually associated with the first intake/assessment appointment, rather than a tab within the client file. I have found that I am writing information on that tab, but then I also have to complete something for the progress note, even though the information associated with that appointment doesn't truly go there. It is frustrating!
