Customize existing clinical note templates
Allow me to customize the existing clinical note templates (i.e. progress notes, treatment plan, initial assessment, discharge summary) so that I can add and remove fields that are relevant to my practice and meet my payer requirements.

It would be helpful to just add a "notes" section in all Theranest forms for copy/paste content and free form as well as being able to add our own spin onto the exiting templates.
This applies to the intake evaluation, the progress notes, and the group notes as well.

After a recent audit, it was suggested that our Progress notes have "more meat." At a minimum we would like to be able to add prompts so clinicians have a clearer picture of what is being required.
The same goes with treatment planning. Being able to edit the treatment plans would streamline our note keeping. Because of the requirements made and monitored by the local managed care we had to develop a separate custom plan, and still utilize parts of the system generated Treatment Plan.

On Strategic Plan, I would suggest changing "Behavioral Definition" to "Behaviors to Work On' or delete that all together

I would really love to ADD fields to the Progress Note and Initial Assessment forms. I see that you have given us the ability to DELETE fields, so that's a good start. If I could add fields I could create a progress note that would be more user and consumer friendly for my practice.

I'm getting on Medicaid panels and this feature is absolutely essential for me to continue using theranest! Medicaid has very specific requirements for notes and those requirements change often so I need to be able to update and customize as needed.

In the Risk area of the Intake and assessment where is has a chack mark area for substance, suicide, abuse...
Consider including relapse into eating disorder, Risk and ton e of the boxes being Eating Disorder.To take it one step further on the progress notes, perhaps include in the area where it says Suicidality, and homicidality, include Substance use, and Eating disorder.

This is the one feature that is missing that keeps me from moving fully to Theranest. I need to be able to customize my progress note templates. It saves my clinicians hours of documentation time.

Allow wording of sections to be changed or other commone note templates to be selected. For example, I use PIE notes (Purpose, Intervention, Effectiveness) and already have wording for each section that meets criteria for audits and would love to be able to plug that in and use it with all clients and just add and edit what I need to at the time of each session.

I find the session note fields (session focus, therapeutic and planned intervention) to be frustrating. I would like to customize what these say or how this form looks to better suite the way I write progress notes. The problem is, if I leave one blank because I wrote it somewhere else it looks like I didn't complete it. I also struggle with the wording used to identify each text box.

Remove Contract/Safety Plan from initial assessment. This feature does not fit with how my agency conceptualizes or conducts suicide assessments of risk for homicide assessment.

I think we should be able to change the layout, sections, etc. of the initial diagnostic assessment. Many states require a particular organized format for evaluations.

add boxes for intake assessment- family history, medical history, psych history, legal, substances, trauma, developemental, sleep, diet, etc.

Ability to add, remove, and edit fields on the progress note template. There are items included on the notes that I would prefer to be worded differently or written verbally instead of check boxes. The options to make a custom form on theranest are too difficult to use to make my own nice looking progress note that I have always used, so I just continue to keep my notes in word documents. I would love to be able to use the full functionality of organization offered by theranest such as being able to create the note right off the agenda and keep better track of it, but I need to be able to use my own template.

Can the initial evaluation form be modified to conform to a particular agency's needs? For instance, some of the fields on the standard form do not apply to our agency. These include the section on appropriateness for agency services.

Since progress notes are needed to do billing. Progress notes should be customizable. I would like to add of features like checkable boxes, pull down lists,etc.

I would also like to customize the initial assessment template. Nebraska requires adherence to a standardized reporting format for all evaluations, and if we could edit the IA, we would be able to all have a more appropriate tool for our practices!
