Customize existing clinical note templates
Allow me to customize the existing clinical note templates (i.e. progress notes, treatment plan, initial assessment, discharge summary) so that I can add and remove fields that are relevant to my practice and meet my payer requirements.

I would like the ability to indicate and edit who was present in a session. Ideally, this field would default to the client, but could be edited to add any additional participants (ie; for couple or family sessions), or change to collateral contacts when the primary client is not present (ie; partners, parents, etc).

I would really like to see the Progress Notes have the ability to add fields as well as delete and edit those that are already there. I work with a number of different insurance companies that have different requirements for progress notes. It looks like many other users agree and would like to see this change as it would truly benefit many of us a great deal.

Feedback after feedback. Month after month...and you haven't responded with a user friendly means for therapists to edit a template and to still import info from pt date and a prior note. I have to say that this lack of responsiveness to your customers' feedback is concerning. I may need to cut my losses and leave for another provider - just as my patients should if they perceive me to be uncaring or unresponsive.

Please allow us to customize- I am not a mental health professional, so I would like to modify your forms and/or upload my own forms that are more applicable to my practice, while still being able to populate existing information such as dx, goals, etc.

I have been using note templates that I like very much through a group practice's EMR. I would like to use the same templates for my patients outside of the group practice but I see that this is not an option with TheraNest. Will this be remedied? It seems this is an essential feature. By the way, with this group practice's EMR, one can easily add and use one's own templates, so it can be done. Thank you!

We need to be able to edit all boxes and headings. I'm a speech therapist, and the titles and format do not fit my needs.

Another lack on the progress notes is that if you create a custom form, you lose all of the auto-populated information such as client details and ID number, service date, session length, provider, diagnostic impressions, treatment plan, etc. If one could add these auto-populated fields to a custom form, many problems with customizing forms would be solved.

We are a body-oriented psychotherapy practice. I would like to be able to customize the progress note to add additional fields providing checkboxes for (user-defined) interventions we use with fields to provide detail if needed. For example, a checkbox for EMDR, with a pop-up (or accordion down) with fields BLS method, NC, PC, Beginning SUDS, Ending SUDS, Begininng VOC, Ending VOC, Body Scan, and another field for optional notes. Another checkbox would be for Somatic Experiencing, with a pop-up (or accordion down) with fields for Tools Used, Touch, and a general optional note. My therapists are asking for this, and I know of another body-oriented group practice in the area using Theranest that would like this, too. You allow for making separate customizable forms. Why not bring those same tools to the progress note? Given the votes and all the comments on making the progress note customizable, this is clearly something that your users want.

PLEASE make these notes actually customizable. It's great that we can now remove things that we don't want. But, it would be so much more useful for us to be able to add fields (like for modality), modify the drop-down options (limit to just the ones we actually use), add a section for misc. notes about mental status & reorganize the fields and drop-down options

I hope to be able to edit progress notes and I absolutely want to format to be something that is aesthetically pleasing and functional. I'd like to see boxes edited for my treatment style.
Any check boxes that might be seen in future versions/updates need to be customizable for ease of use. What a DBT therapist would want will be different from speech/occupational theray... Maybe these can be on/off for user.
I find intake forms have a lot of sections that don't fit my treatment approach, It'd be great to have the health care section to be expanded on so it becomes a whole section for any other treatment team professionals....group therapist, psychiatrist, neurologist, school counselor...
I'd like to see diagnoses more easily transferable between types of treatment....I run a group so often diagnoses disappear and have to be redone over and over...maybe cases can be linked or not linked for different cases. I suspect they might not want to be linked sometimes?
If forms can be partially completed and sent through portal for signature, it'd increase ease of use for things like ROIs and med updates, etc. The more electronic...the better!
And then if we can copy/paste updates with ease....what an amazing improvement... -

We use a custom form for this, it works really well! It would definitely be useful to have all of the note in one place, but I bet it'd be tricky when printing notes to only allow "clinical fields" to print.

Create a place at the bottom of the note that allows user to document therapist notes that are not included in the clinical record.

Instead of having the big boxes for the answers to questions (which are usually very short), down at the bottom, the boxes for what is overdeveloped, underdeveloped and how the client will react to pain need a lot more space as there is a lot of info that needs to go there. A small box that's maybe and 1x1 does not work.

it would be really helpful if the notes section could be edited in a more specific manner and then saved as a template. As a therapist working with children I need a different format.
