Customize existing clinical note templates
Allow me to customize the existing clinical note templates (i.e. progress notes, treatment plan, initial assessment, discharge summary) so that I can add and remove fields that are relevant to my practice and meet my payer requirements.

Allow the initial assessment and diagnosis section to be customizable so we can tailor it to our specific practice like editing the sections names and or deleting certain sections.

Absolutely would love the ability to customize the note. The current note meets the bare minimal needs, but some insurance companies want different information, so it would definitely be helpful to be able to customize to prepare for a possible audit.

The treatment plan form must include additional information in order to be compliant with specific insurance plans. Currently, I am having to add this information to sections where it just doesn't belong in order to remain in compliance (though, I haven't yet been audited).
I don't feel a treatment plan made the custom form creator will appear as organized as the current form.
Is it possible to give us the ability to add or remove sections from the treatment plan or make the custom form optional more visually appealing?

Initial evaluations and progress notes can significantly vary depending on the clinician's background and training. TheraNest should allow clinicians to create custom note templates that can be used in place of the default note templates. Although custom templates can be created in the Custom Forms section, going that route means having to create two notes per encounter (i.e., a TheraNest default note plus a custom note), which is an undesirable option.
Also, when I write my notes as a psychiatrist, I really don't like being forced to include the therapist's Treatment Plan notes in my clinical notes. I should be able to create a note to suit my needs as a physician, just as clinicians with other backgrounds should be able to do the same.

It would be great to add a section for non-billable notes or info notes. This would be different than the existing "client notes" that you offer. We should be able to use other templates such as GIRPP notes or our own business style notes.

Yes, I cant believe wasnt an option from beginning to have the ability to adapt the Progress note section Headers to how people may have been trained or doing them for years: a SOAP note or a GIRP note or whatever section name one chooses.

Please, please please create a means by which we can create a custom progress note.... even if it is a blank form...There are other things that we do besides just therapy. This is me begging......

This would be SO helpful, especially with necessary documentation due to COVID19. The "customize" your own is great, but that still leaves open progress notes that could use updates!

If there is a way to customize the boxes to meet the needs of our clinic. ex we dont needIs Client Appropriate For Agency Services? And we also would prefer check box options for treatment length (5-7, 8-12, 12-15 sessions) or pertinent history ( we would put check boxes like has been in treatment before, married, divorced, etc). It is really limited to what the theranest template has deemed appropriate fields for the initial assessment criteria.

I would like to be able to change the Initial Assessment & Diagnostic Codes template to include the following information, which is required by my and other states.
Past Psychiatric History: (Obvious reasons)
Trauma History: (Obvious reasons)
Family Psychiatric History: (I've been including this in the Family/Psychosocial Assessment
, but it would be great to have a separate section for it).
Medical Conditions & History: (some medical problems can have serious psychiatric side-effects)
Current Medications: (needed to be aware of psychiatry and possible side-effects of medications)
Substance Use: (I am a dual diagnosis therapist and NOT having this section seems wrong to me)
Social History: (how a client is away from their family can be as important as with their family)
Developmental History: (Needed for better understanding if there are medical connections or if there were problems early on in life)
Legal History: (if legal problems, are there psychiatric reasons?)
Strengths / Liabilities: I see the strengths, but liabilities can be important information as well - for example, Low SES can be a huge indicator of possible trauma given living situations) -

We should be able to edit the progress note template to fit insurance billing needs. Simple practice allows for this and I am currently considering a change to them as it would be less work.

Each state and insurance company has requirements that need to be followed by each provider. It would be great to be able to ensure we are in compliance by being able to edit the templates provided by theranest ie. progress notes, assessment, treatment plan, so we can do so. Many other provider system allow this such as simple practice.

It would be helpful to be able to add custom fields to our note template to make them personal to the practice. Including the ability to have check boxes to be able to check certain things off that are present during the time of session.
