Remove "undefined" option from legal sex field

When filling out a client's information, remove the "undefined" option from the Legal Sex field.

    Undefined should not be a legal sex option and insurance does not recognize this. Since this is what populates on the claim, this option needs to be removed and only allow clients to choose M or F.
