Add AUDIT as an intake form
Have TheraNest add the AUDIT to the list of intake forms so that I don't have to create it myself.

Load the Columbia Suicide Severity Rating Scale (CSSRC), the Alcohol Use Disorders Identification Test (AUDIT), and the ADHD Self-report Scale (ASRS-5) to the intake/notes section.
