The following forms have been added to your organization’s Dynamic Forms:
- Good Faith Estimate
- Release of Information: HIPAA AUTHORIZATION FOR USE OR DISCLOSURE OF HEALTH INFORMATION
- Release of Information: Additional Consent for HIV/AIDS
These forms are currently archived. You’ll need to unarchive them in order to use them at your organization.

Andrew Last commented I agree! Why hasn't this happened yet? It's mandated by law and for those of us who only do intake paperwork through theranest it's a pretty big issue to have to send our clients a pdf copy via email. Please add this to the portal so we can utilize asap!

Amanda Cosel commented I am really surprised this hasn't been implemented by Theranest yet. This is now required by law, and it has been almost a month since this was implemented. I have been such a proponent of TheraNest and have never ever considered using any alternative EHR system. It seems TheraNest is typically comparable to Simple Practice. Simple Practice incorporated this document immediately after this became a requirement by the law, making it seamless to submit to clients and customize per client. Although we are able to create our own dynamic forms, this feature in TheraNest is very user unfriendly and not nearly as intuitive as the rest of TheraNest. It would be of GREAT benefit for TheraNest to create this form for its users.

commentedI was wondering if you all will be adding the good faith estimate/no surprise fee act to your dynamic form templates for providers to send to our clients since this is not required by law. That would be a great addition to the current great forms you all provide. Here's the link with more info on the specifics of the new law and I've attached the pdf of the new law. https://www.cms.gov/nosurprises/consumers

commentedWE could use the intake template but it needs additional elements--dx, date of first session, client address, CPT codes...

commentedCreate a template that follows CMS template and can be edited but he provider to specify client information such as DOB, home address etc as well as estimates for services but cannot be edited by client. Also a space for the client to sign the document and send it back with the rest of the intake forms so everything is returned. Thank you!

commentedAs clinicians we must be in compliance with CMS mandates and this is now effective for 2022. This is required before seeing the patient. It must be a "separate document" that all clients receive so we can't just add a text box to existing documents. Thanks!
