Customize portal invite message for contacts in contact network
When I add an individual to a client's contact network, allow me to customize the portal invitation message they receive.

Please update this to allow us to choose intake paperwork to be sent during initial portal invite. It is very clunky to use for parents who need to complete paperwork for a minor.

Customizable invitation messages and form selections should apply in cases where only the contact of a client would have portal access. It should apply to the first person to access a client portal account regardless of if it's the client or a client's contact setting up and logging into a portal account for the first time.
