Remove organization address from intake forms
Remove the address from the intake forms (e.g., informed consent). I only practice telehealth and I don't want my home address appearing on the form.

commentedThis issue needs a solution urgently. I am also trying to start a small telehealth private practice from my home and Theranest puts my location address (which is my home address and to which my business is registered) on the top of the form clients will fill out. I do not want clients to have my home address. What is a work around solution in the meantime?

commentedAllow an option to skip main practice address and phone on intake forms and forms in general. Virtual practices dont want their home address listed on the forms. It is already skip-able on the superbill with the location feature. The forms is the only place that Theranest is not allowing me to keep my address private.

Sean Hershey commented I have a small telehealth private practice that I run from home. Theranest puts my address (which is my home address) on top of the forms I send to clients. I really don't want clients to have my home address, but the theranest team tells me that this is the way the system is. Can we please change this for those of us who have telehealth practices? I really don't want all my clients to have my home address.

commentedPlease do not make addresses populate automatically onto intake forms, statements, etc. Those of us doing telehealth may not have an address other than our home address. If not billing insurance, there is no need for clients to have access to a physical address for the practice. Thank you.
