Make statements more visually appealing

Make the statements more visually appealing. Right now they are very grey and bland.

    We also need to see the billed amount per service date as far as insurance is concerned.

    I agree with this. I would also ask that the spacing in the header be decreased. Right now many of my statements fall to two pages when it could easily be one page.
