Release of information intake document
Have TheraNest create a Release of Information document that's included as part of the intake paperwork.

We really need a way to create custom forms for clients to sign electronically, since so many of us are doing telehealth. A ROI needs to have information specific to a client (their name, who the release is for, etc), which is difficult to do with the intake documents.

It would also be helpful for the therapist to be able to electronically sign forms (such as ROI) after the client has signed. Since we are doing everything digitally right now, it would be great if the client could sign their ROI and have the therapist sign it after without needing to print it off.

I would like there to be a feature with a basic consent form that can be sent to the clients through the client portal giving permission to release information to parents, other providers etc.

I use a general Release of Information form with all my clients. Here it is below. It would be terrific if Theranest made something similar that I could check off and send to patients through the client portal with all the rest of the intake paperwork. I could make a custom form, but I'm guessing many providers would benefit from having a form like this. Maybe Theranest could format a form and have it available for us to all use.

This would be SO helpful. These are the kind of forms that are so difficult to add as a custom form due to the complex formatting. Please make this a priority.
An alternative, is there a way to allow us to upload a PDF of a form and have a client interact with it?

Please provide a template whereby clients can electronically complete and sign a release of information which can be added to their file. I hear that Simple Practice offers this.

Create a tab for Release of Information under each client. Include fill-ins for who to release info to, the date of the release, and the date it is good through/expire date of the release.

Have a specified area (maybe) within the clients details area that they have filled out a release of information to specific people, doctors, lawyers, etc. *At this time we use the very last portion, at the bottom of the Client Details listed under Managed Household (I believe that's what it's called), which is a great alternative as it allows you to add several people. At this time we do use that area for this purpose as we will list both parents (if the client is a child), a doctor, etc.

There should be a tab in client files that lists all those who are authorized to have information released to them that includes their relationship to the client and DOB. It would be extremely helpful for when a parent calls to inquire about a child's appt. to have quick access to verify whether or not we can give out info.

I'd like to 2nd this...or at least a form that we can customize that would allow a patient to complete with me and then sign off on it.
