    We keep getting insurance denials due to not completing Box 14 on the 1500 form, mainly with BCBS. We researched 1500 forms and below is what we found:

    Box 14 Required Date of Current - Illness (First Symptom) OR Injury OR Pregnancy (LMP) - Enter the date of onset of the member's
    illness, the date of accident/injury or the date of the last
    menstrual period

    Box 14 is required to be filled out but currently on Theranest there is no option for it to be filled out automatically. We have to print out form and edit it and then mail it to the Insurance company. We are requesting that Theranest have an option for Box 14 to be filled out automatically, that we really help out. Thank you.
