The characters in the Subjective section of the SOAP note should not be limited. This could cause claims to get denied because important information that the client says or that is seen cannot be but into the note because of the limitation. This is particularly difficult for nurse practitioners and providers who are prescribing medications and needing to validate diagnoses.
