Upload existing documents to create dynamic forms
I have all my own forms for my clients already made into word documents and PDFs. Allow me to upload these into TheraNest and use them as dynamic forms so that I don't have to recreate them.
Unfortunately the technology currently available to support this requested functionality is not advanced enough to allow this type of import to work effectively given the needs of our customers.

I would also like to see this feature added to TheraNest. So much faster than trying to re-create all the forms within the system! Thanks for considering this - it would make the work of the individual practitioner who has to "do it all" much simpler and helpful!

The ability to upload forms I have created for my practice is a MUST! I have spent many years working on my paperwork and it only makes sense to be able to upload it! It does not make sense to have to recreate pages and pages...and it is extremely difficult to do in Theranest. Definitely not user-friendly software.

Ability to upload your own forms to be sent out through the client portal. I have my own forms created and am not able to send them through the portal.

I would like it if I could upload my assessments and Service Agreement Plan along with other HIPPA compliant forms instead of having to remake them in the upload portion of Clinical documents. Is this a possibility? This way everything could be in the electronic record instead of in a floppy paper file.

Hi, at our organization we need custom forms to be uploaded for use. Some of our forms, like informed consent and release of information are created with the City Attorney and cannot be substituted. Please address this issue. Municipal entities like ours have specific legal bodies to work with, serve litigious clientele, and therefore, necessitate the use of approved legal documents. Also, given the unique stressors of our clients, specific intake forms with taylored questions are needed and the intake template builder is not as user friendly as a word document. Also, adding our organizational logo is not possible with the template builder. Please address this otherwise we may need to take our business elsewhere.

I'm still exploring other EHR's and am considering transitioning to a different one because this feature has not been added to Theranest yet. It appears that this is an option on both SimplePractice and Therapy Notes :(

I would love if it were easier to upload and use our own templates for notes, intake forms, treatment plans, etc. I have already designed the wording and layout that I prefer and would like to be able to just upload and use the forms I already have and keep everything organized and stored on TN. The document creator option requires documents be too simplistic and unformatted and the TN templates do not use the wording and categories I need.

There should be a way to upload intakes, progress, IM+CAN, etc. Via Word, PDF, ect. Its hard to create the proper forms that is needed when conducting services. I think that is pivotal private practice and before subscribing with a big company.

It is way to time-consuming to edit and personalize the intake documents currently available in Theranest.
I already have spent a lot of time and money creating my own intake paperwork with logo, etc. and would love a one-button "Upload" feature so that I can easily get those to clients. It is the one reason I refuse to use the client portal feature.

Allow for PDF integration, the ability to upload fillable forms to the patient portal to be sent for filling and for signature.

Yes! This would help so much for being able to actual get the full use out of theranest for progress note taking, treatment plans, assessments, etc. I just need to be able to upload my existing templates and fill them in for each client session. The progress note "customization" at this time only allows for hiding or revealing the predetermined sections which are not worded the way I prefer, aren't necessary, or don't include sections that I do feel are necessary. At this point, I keep all my documentation separate and only use theranest for billing which is a shame.

I'd really like to be able to upload specific assessments to the client portal and not have to create them.
Otherwise I have to make a separate email and attach them which is an added administrative task. It then requires me to upload their completed assessment (s) to their general documents. I dont want to save these separately or send out separately. It makes for a very fragmented process and ient file. -

I have already created documents I would like to use to send out to new patients with the portal invitation. If there is an option already to send out my existing documents for e-signature, I have not been able to figure that out. I like that you can make customizable documents on your portal, BUT, it is a hassle if you have already created the document and are just trying to import it. I've included an example of what I have already created, but it seems like my only option is to copy and paste it into the customize a document section ... and then it just messes it all up.

We would love to have an option to add personalized client documentation, such as intake forms, signed consents, and assessments, into the system and the option to have clients sign these electronically.

YES!!!! If we can easily upload client documents that are pdfs to their cases, why can't we easily upload pdf forms to client portal so that they are fillable and signable?

if I've already made custom forms elsewhere its really hard to make them again from scratch. We make an IEP form for each child so I also wonder if forms can be client specific.
