Add rating scale as a dynamic form element
Allow me to add a rating scale as a dynamic form element where the ratings appear horizontally.

Create a slider form element where there is a way to enter the limits of the slider (e.g. 1 - 100) so the person can grab the slider and drop it somewhere on that line, and it will show the person which number they are landing on (e.g. they drop it at 73).

Add a rating scale or likert scale element to dynamic forms. For example, "Please rate your overall well-being on a scale from 0-10." Or, "Please indicate how much you agree with the following statement," etc.
