Add ethnicity field to client details

When filling out a client's details, allow me to add their ethnicity as well as their race.

    According to the US census, Latino/a is a race and Hispanic would be an ethnicity.

    Make Hispanic separate from race as Hispanic is an ethnicity not a race.
