Automatically populate city and state with the zip code

Have the client's city and state automatically populate when I type in the zip code.

    With the last 4 digits of Zip Code also. Insurance rejects this without the full 9 digits.

    Yes, especially ZIP Code with the last 4 digits. It keeps getting rejected by insurances and we have to look up each zip code on line
