Hide practice phone number on intake forms
Allow me to not display my practice phone number on intake forms.

On the upper righthand corner of intake paperwork the organization name and phone number are displayed (a phone number is required even if it is a personal number). There is no option to not disclose phone number or provide a business email address instead. It would be helpful to have the option to either not show phone number or provide a different source of contact information like an email or website instead.
