    When patients receive a packet of forms numbered 1, 3 and 4, for example, they naturally assume they are missing form 2. Removing the form number would help this. Alternatively, it would be OK with us if the entire form name was left off. We can always include that in the document itself.

    In the message that is sent to clients when they're invited to the portal we have had to include the text "PLEASE SCROLL TO THE BOTTOM OF THE FORM AND CLICK THE SUBMIT BUTTON" because this is so confusing. For us, the functionality to save the signature is unnecessary and only compounds the problem. If the submission page can't be changed (although it seems like pretty simple programming to do so), it would be at least helpful to prompt the patient that they are not finished if they attempt to leave the final screen without submitting. The point of the entire exercise is to collect their forms, not log their signature.
