Add a table to a dynamic form
Allow me to add a table to a dynamic form so clients can fill in parts of the table.

I agree. I really need a table to be able to keep all information in one area and organized better. Please consider this as soon as possible.

Our forms often have extensive areas of simple checklists that take up too much space- sometimes three pages!! Two columns do not fit the rest of the form, so a table would really make things look a lot better and drastically shorten some forms with checklists.

ADHD scaled form. It would be nice to be able to create scaled forms like the ADHD self report scale so that can be sent through the client portal. See the form attached.
