Add Mental Status Exam to the initial assessment

Add a Mental Status Exam to the initial assessment template in Theranest it is the only area missing to be Medicaid compliant.

    same as the standard note, add it to the initial assessment.

    Please add a mental status section to the initial evaluation, and make it possible to connect the initial evaluation to the initial appointment. Theranest is prompting me to complete a progress note when I have with the initial assessment.

    The mini-mental status exam should be included in the intake note template and not just in the progress note template. Should be in both?!
