Formatting options for dynamic forms
Would like the ability to format custom forms similar to other word processors:
- Adjust font sizes, colors
- Paragraph styles
- Inserting tables
- Adding images

This is a necessity! My diagnostic assessment (custom form) is 10 pages due to being unable to format the custom form in a more effective manner.
I would also like to an option that if a box is checked yes that it autopopulates a box that requests more info.
Add the option to add an addendum to custom forms.
Add the option to autopopulate specific fields beyond client name, etc. But to also include the option to autopopulate treatment plan goals/objectives onto the form if desired.

1) Add the ability to make the forced choice questions to be required to respond to.
2) Remove the restriction on having the same text not allowed. For gathering data from forms, this makes sense, but for an intake questionnaire where you may want people to explain yes responses several times, etc., it would be helpful to not have this always enacted!
3) make it possible to enter questionnaire type items. -

During the intake process, we draw a genogram for our clients, and it would be helpful to have a field in the form where we could attach a scan instead of needing to upload it separately in the General Documents tab

I would like to have the ability to put multiple checkboxes on a row on an custom form to help reduce the length of a form. Also the ability to split the form for certain sections of the form, rather than the whole form. For a document like a Comprehensive Clinical Assessment, the checkboxes are tedius, but necessary for the assessment to remain in standard time. Having the ability to put them on rows/columns just creates a shorter document.

It would be wonderful if the check boxes weren't so separated down a line, but if they were grouped together so the form wasn't so long. Also it would be nice if we could enter multiple drop down options at once just like you guys created in the mental status exam page.

Create customizable drop-down menus for demographic info, including county, state, etc.

Please add a default "--" or "select below" to drop-down lists.
Please add radio buttons instead of just drop downs and checkboxes, as they are far more intuitive to most.
Please add more options for headers/section titles, like font size.

Having an option on custom forms where I can make a fill in the blank. For example, I want to create an agreement form that is something like: I ____ (full name), authorize Dr. TheraNest to charge my credit card for the amount of ___ (amount).

I've used the option to create my own form, but the outcome was very disappointing. The final product should at least have had even a few more of the options available in the other notes in your system. For example, each page should have a header with the identifying patient information populated on the top. The option to customize headers or footers would be helpful. Also, the text that one enters needs to be able to remain formatted at typed for ease of reading, rather than jammed together into one run-on paragraph. WYSIWYG.
Another option would be to allow therapists to import documents from MS Word into a case file. Even more useful would be if forms that were created could be sent to a device such as an iPad via bluetooth, allowing for a signature from a client, and then saved within the file.
Improving this feature would go a long way toward allowing practitioners to truly say TheraNest allows for a paperless practice.

It would be nice if the forms that we can create had more flexibility. currently where I can place my sentences and answer boxes are limited to a basic formate.

I hate to have to re-do all my forms etc- tone table to change font/size/ or be able to copy and paste instead of having o remake and figure out header vs paragraphic

If we could just, in the interim until a more robust solution is implemented, the ability to keep some simple formatting from a copy/paste, that would be a huge improvement.
For example, just allowing a carriage return (new paragraph/space) would be a big deal since it would saving having to create a new element for every new paragraph. -

Yes please do this soon. My older clients have a very difficult time reading the tight font.

Great comments on here. Thanks to everyone that has taken the time to make suggestions.
I agree with most of them and do feel that the Forms interface could be improved.
I suggest looking at SurveyMonkey. They have a slick interface that's user-friendly - easy to learn and with a lot of customization. I also like the idea of the "Floating" menu bar, or a static menu bar in a margin so you can choose your element and immediately start working on it without having to scroll up and down.
The ability to insert tables with columns would be very useful as I'm sure many have forms with tables embedded and it really streamlines the intake process for both client and therapist.
The current system is very tedious for formatting and there should be some WYSIWYG components incorporated for improved ease of use. Thank you! -

When creating a custom form, the field (comments) can you make it so we can do paragraphs within the field? It is creating a run-on paragraph and doesn't look good.

I work from a private residence, and so I would like to add a jpeg photo of the front of the house to my intake forms to aid my clients in finding the right location of my office.
